Appointment booking
All appointments with Dr Jamnagerwalla are booked in advance. Appointments may also be requested via our Appointment Request form.
For new appointments, please try to arrive a few minutes earlier as you will be required to fill in a New Patient Registration form. This can also be completed online via our New Patient Registration form. If you are running late for your appointment kindly inform our reception staff.
Your first appointment with Dr Jamnagerwalla will be between 15-30 minutes. A history relating to the purpose of your referral will be taken, followed by an examination in most cases and a discussion regarding possible diagnoses. Any further investigations required by Dr Jamnagerwalla, e.g. blood tests, CT scans or ultrasounds, will be advised and treatment options discussed.
Appointment cancellation
Prior to your scheduled appointment, an SMS reminder will be sent to you. If you are unable to attend your appointment, reply by texting “N”. Please notify us as early as possible if you are unable to attend your appointment as we may be able to bring appointments forward for other patients.
Preparation for your consultation
Please bring the following items with you to your appointment:
- A valid GP referral, if one has not been sent to us prior (fax, email, online form).
- Medicare Card.
- Private Health Insurance card or number if applicable.
- Any x-rays, scans, blood tests or other relevant documentation relating to your problem.
